LICENSlNG AND ACCREDITATION
The Levittown Beauty Academy was established in 1964, and licensed by the Pennsylvania
Department of State, Commissioner of Professionanal and Occupational affairs, State Board of
Cosmetology, and is empowered by that authority to train and certify students for examination in all
licensed Beauty Culture Arts and Sciences. The Academy is nationally accredited by the National
Accrediting Commission of Cosmetology Arts and Sciences.
The Academy is approved by the Pennsylvania Department of Education for the training of
veterans and other persons by the United States lmmigration and Naturalization Service for foreign
students. Further approvals include: The Bureau of Vocational Rehabilitation The, Bucks County
Office of Employmenat and Training and the Commonwealth of Pennsylvania, Department of
Labor and industry , Bureau of Employment Services and Training , Trade Acts of 1974 and 1981.
Member of the Association of Accredited Cosmetology Schools.
RIGHTS AND PRIVACY ACT PROVISIONS
Each year, The Levittown Beauty Academy informs students of the Family Educational Rights
and Privacy Act of 1974.
School records regarding a student's personal data, hours, financial payments and progress
and / or achievement records will only be released on written instructions from the student and or parent or guardian if student is a dependent minor.
Any student who requests the opportunity to review his or her records will be given access to all
records pertaining to the requesting student , as often as the student wishes.This inspection will
be supervised by the school's director, supervisor, or a teacher. No student will be given the
opportunity to review the records of other students.
The school will also maintain a common federal aid folder by federal year containing student Pell
applications and ISARS o r paper SARS and all loan information by federal year. During periods of
audit or review, student records will be made available to the auditor or reviewer.
To comply fully with the provisions of the Federal Drug Free School Act of 1988, and with the
Federal regulations regarding Campus Security, Levittown Beauty Academy has available its Drug
and A lcohol Policy and related counseling information and its Campus Security Report which is
issued to each student at enrollment, and on an annual basis.

The Levittown Beauty Academy, founded in 1964, has fully prepared thousands of graduating students for careers in Cosmetology and the Field of Beauty.
Levittown Beauty Academy is a private comprehensive school licensed by the Commonwealth of Pennsylvania, Department of State, Commissioner of Professional and Occupational Affairs, and the PA. State Board of Cosmetology for the training of students in all licensed courses including: Cosmetology, Manicuring, and Cosmetology Teacher. The academy is nationally accredited by the National Accrediting Commission of Cosmetology Arts and Sciences. Additional approvals and professional memberships can be found in other areas of this catalog.
The Academy's courses are prepared, structured, and presented so that each student receives a well-rounded education in the Cosmetology Profession.
Our professional teachers and staff use up to date modern techniques and equipment for instruction. Courses are designed with today's hairstylists and cutters in mind. In our Customer Clinic, students develop and practice skills on patrons, so they are able to obtain immediate employment in the field of Cosmetology.
Each of our staff and instructors has had many years of experience in the field and as educators of Cosmetology. Since they have owned and managed Beauty Salons,they are also competent in giving practical business instruction so necessary for the successful operation of a business.
For more than 40 years the Academy has been dedicated to the teaching of students in a technical, professional, and ethical approach to the profession. This spirit carries over to the faculty and staff of today's Levittown Beauty Academy,
where diverse backgrounds unite to carry out the objectives of the Academy with an atmosphere of respect to the individual.
The Levittown Beauty Academy is located in Levittown, Pa. which is a suburb of Philadelphia, close to Trenton, New Jersey. The Academy has approximately 6400 Sq. feet of learning area.
The Academy has a Junior Clinic where beginning students learn and practice their skills. This area is fitted with all the equipment and materials used for Junior instruction. The Academy also has a Senior Clinic , where advanced students practice and enhance their skills on patrons. Services in this area are conducted in the professional and ethical atmosphere of a Beauty Salon. Included in the Academy are theory class rooms where instruction in the theoretical phases of Cosmetology are taught. Blackboards and projection and video equipment are located in this area.
The Academy has easy access to all handicapped persons.
A career in Beauty offers many rewards. There are excellent financial returns because this profession is rapidly growing, it is the third largest industry in the country. Trained Cosmetology Professionals are in demand.
The Levittown Beauty Academy's students have the opportunity to specialize in many areas of the Beauty Industry.
Platform Artists and other guest speakers and educators visit the school and conduct specialized demonstrations and lectures. In addition to cutting and styling, other areas of specialization available are Coloring, Permanent Waving, Hair and Scalp Treatments, Facial and Make-Up Artistry, Manicuring, Salon Management and Teaching.

As in classic cars, there are classic lines to hairstyling, cutting, and all artistries of beauty. Even though specific looks may be different, similarities can be found in the underlying design. The Academy is very conscious of the link between past, present, and future trends. The School prepares its students in all basic and modern theories and techniques so that as fashion trends change our graduates have the knowledge and skills necessary to change with the times and create the latest looks.
A BEAUTY PROFESSION can be creative for the imaginative individual and is always fulfilling because one can see the end results of his work. There is a lot of contact with people and styles are always changing so as a job it is never dull. ADMISSION REQUIREMENTS
COSMETOLOGIST and NAIL TECH COURSES:
The academy admits as regular students only persons who have graduated from a school providing secondary high school education, or the equivalent (GED) or admits as regular students persons who are beyond the age of compulsory school attendance, and have the ability to benefit from the training offered by the school. For students who have not graduated from high school or met the equivalent requirement (GED), the school will have administered, by a private tester, a test which is approved by the United States Department of Education, for this purpose.
COSMETOLOGY TEACHER'S COURSE:
The academy admits applicants who are 18 years of age or over and have graduated from a school providing secondary education, or the recognized equivalent (GED), and have completed an approved cosmetology program.
UNIFORM DRESS CODE
Levittown Beauty Academy's students wear black slacks with any color blouse, shirt, or sweater along with a black tapered smock monogrammed with the initials "LBA" and closed toe shoes. The black monogrammed smock may be purchased from the school if the student wishes, or it may be purchased from a beauty supply store of the student's choice.

Tuition deposits are required at the time of enrollment, when each student signs the enrollment contract. This deposit is subtracted from the total tuition cost.

Students eligible for Title IV Funds will have these funds applied to their tuition accounts and their payment plans adjusted accordingly.

REFUND POLICY
a. An applicant not accepted by the school shall be entitled to a refund of all monies paid.
b. If a student (or if a student is under the legal age, his/her parent or legal guardian) cancels her/his enrollment and requests money back within three business days of the signing of an enrollment contract, all monies collected by the school shall be refunded. The cancellation date will be determined by the postmark on the written notification, or the date the notice is delivered to the school administrator or owner in person. This policy applies regardless of whether or not the student has actually started training.
c. If a student cancels her/his enrollment after three business days after the signing of the contract but prior to entering classes she/he shall be entitled to a refund of all monies paid to the school less a registration and processing fee of $100.00.
d. For students who enroll in and begin classes, the following schedule of tuition adjustment is authorized:

e. In case of illness or disabling accident, death in the immediate family, or other circumstances beyond the control of the student, the school shall make a settlement which is reasonable and fair.
f. Should the school permanently close and no longer offer instruction after a student has enrolled, the student shall be entitled to and will receive a pro-rata refund of tuition.
g. Should a course be cancelled subsequent to a student's enrollment, the school will refund all unused monies paid.
FINANCIAL AID...
FEDERAL PELL GRANTS
The Pell program is a federally administered program available to eligible students. Students may receive up to $4310.00 per academic year of 900 hours, or up to $5986.00 for the 1250 hour program. Applications are available from the school office. Eligibility is determined by the federal government. In addition to financial need, a student must make satisfactory academic and attendance progress. The school's director can help you determine your eligibility for this grant.
OTHER FINANCIAL AID
Various agencies offer needy students aid toward their education, and criteria for this aid can be obtained from the school office.
SATISFACTORY PROGRESS
In order to remain eligible for federal aid, all students must maintain satisfactory academic and attendance progress. The school's satisfactory progress policy is published and delivered to all students.
FFEL STAFFORD LOAN
Stafford loans are available through the Federal Family Education Loan Program. This program is administered by agencies designated by the U.S. Department of Education, and participating lending institutions. Students enrolled on at least a half-time basis and making satisfactory progress, may apply for this program. The yearly limit is: $7500.00. The current interest rate is variable. Repayment begins six months after graduation or when the student ceases half-time enrollment.
APPLICATION PROCEDURES FOR FINANCIAL AID
All applicants for financial aid must file the Free Application for Federal Student Aid. These applications are available from the office. As long as there is continuing evidence of satisfactory progress, as well as financial need, the Academy will make every effort to assist the student in obtaining financial aid.

RETURN OF TITLE IV FUNDS
Students who withdraw before the program completion and receive financial aid in the form of Federal Student loans and/or Pell funding are subject to the return of Title IV funds. The amount of the federal refund is determined using the number of completed clock hours verses the number of hours scheduled for completion. Funds will be returned based on the refund Distribution Policy of the Federal Government.
STUDENTS PURSUING A NEW JERSEY LICENSE
Students may upon completion of the Cosmetology program take the Pennsylvania State boards. After successful completion of the Pennsylvania boards the student may take and need to pass a short shaving exam in order to become licensed in New Jersey. Another option for potential NJ Students is to take the 1250 hours certified by the Levittown Beauty Academy, to a school in NJ and have the hours from our Pennsylvania school qualified by a New Jersey Licensed school so you may sit for the New Jersey exam.

CLOCK HOUR POLICY..
The number of clock hours required for each course offered by the school and listed in this catalog is in full compliance with the actual clock hours required by the Licensing Laws in the State of Pennsylvania.
Students receive all actual clock hours they train while the school is in session, and these hours are applied to the total clock hours reported to the Pennsylvania Cosmetology Board.
The Academy does not deduct from or add to the total accrued for any reason.
The school does not assign additional hours for any reason, without exception.
GRADING SYSTEM..
Written examinations are given for each subject. Possible grades which can be achieved are: A-100-93% B-92%-85%
C-84%-77% D-76%-70%
F- Failing - below 70%
Practical and Clinic procedures are evaluated as follows:

REQUIRED LEVEL OF ACHIEVEMENT..
All students are required to achieve at least a 70% theory grade point average, and a S satisfactory grade evaluation in all practical and clinic procedures. All senior students are required to perform a minimum number of clinic services, to qualify for graduation.
ATTENDANCE. POLICY..
Students must come to class on time and attend 70% of the classes offered to maintain satisfactory progress. Situations beyond a student's control, such as accident or illness, will be addressed by the director to determine whether an absense is excusable.
Any student arriving past 9:15 will be considered tardy and must report to the director.
Unless situation is deemed an emergency, the student will not be permitted to enter class.
CLASS STARTS..
New classes begin the first Monday of each month.
SCHOOL HOLIDAYS..
The school is closed on the following holidays: New Years Day, Martin Luther King's Birthday, Good Friday, Easter Monday, Labor Day, July Fourth, Memorial Day, Thanksgiving Day, Christmas Eve (Night Program), Christmas Day, and December 26.
In the event classes must be cancelled because of weather or other any reason, an announcement will be made on the radio on WBCB 1490 FM and FoxTV WTFX and Channel 10 NBC. The recorded information message on our telephone will also give emergency information.
REQUIREMENTS FOR GRADUATION..
Each student must complete the required number of hours for his particular course.
A student who has completed the required number of hours of study in his or her course and has maintained the academic standards set forth, by the Academy, and included in this catalog, and has satisfied all financial obligations to the school, will be awarded a graduation diploma in his or her individual program.
STATE EXAMINATION..
Each student who has completed his course of study and graduated is required to take the PA State Board Examination given by the PA State Board of Cosmetology. The State Test consists of a practical and a written portion. Applicants must take a mannequin to the test. All courses prepare students for this test. Teachers do mock State Board Testing throughout the programs. The Pa. State Board of Cosmetology will issue a cosmetology license to students who pass.
MAKE-UP POLICY..
Students are expected to graduate by the completion date on their contract. Students who need to make up hours may do so at an hourly rate.

COURSES and PROCEDURES...
CREDIT FOR PREVIOUS AND TRANSFER TRAINING..
The Levittown Beauty Academy will give prior academic and tuition credit to students for all hours that they previously trained if all of the following conditions are met:
1. The previous training was in an accredited cosmetology school and the last date of attendance was not longer than 4 years from the date of application with LBA.
2. LBA is able to secure a transfer of these hours from the previous school.
PLACEMENT AND EMPLOYMENT ASSISTANCE...
The Levittown Beauty Academy offers its graduates (regardless of date graduated) assistance in finding employment. The Academy maintains a list of employers who have asked the Academy for graduates, and refers students accordingly. However, the Academy cannot and does not guarantee employment or a specific wage.
COMPLAINT PROCEDURE...
Any student or interested person who has a good reason to believe the Academy has violated any Federal, State, local, health or safety rule or regulation, or has been in violation of NACCAS Standards and Criteria may file a formal complaint with the appropriate agency. The school office will provide the complaint forms, procedures and the addresses necessary for this procedure.

POLICY All students are required to conduct themselves in a manner which is in accordance with the school's rules and regulations. These rules and regulations are described in your enrollment agreement. Please read completely this section of the agreement before signing.
These rules and regulations have been developed to insure that the program offered by the Levittown Beauty Academy are instructive and rewarding for all.
POLICY Should the school determine that a student has demonstrated a pattern of conduct which is contrary to goals of the school in regard to the student body, this student's enrollment will be terminated.
POLICY Release of and access to student records
The school complies with all provisions of the Family Education Rights and Privacy Act of 1974.
School records regarding a student's hours, financial data, progress or achievement record, and any personal data will only be released or provided to a person or agency upon written authorization from the student, for every instance separately. Official auditors and reviewers will have access to records when necessary.
A student may review his records at any time he wishes under the guidance of the school's supervisor or designated school official or teacher.
MINIMUM STANDARD PROGRESS POLICY...
The following is the program length and maximum time frame of each course offered by the Levittown Beauty Academy. In order to make satisfactory progress, the maximum time frame is 150% of program length.

The following Progess Policy has been developed to ensure that all students meet the minimum academic and attendance progress standards necessary to receive an effective, strong and marketable education:
ATTENDANCE PROGRESS POLICY...
Students are required to attend 70% of their scheduled classes. Students unable to adhere to this policy will be considered - "Not Making Minimum Standard Progress."
ACADEMIC PROGRESS POLICY...
All students are required to maintain a minimum academic and practical level of performance in all subjects, equivalent to a 70% grade average. Grade point average is based on the cumulative average of tests, and on the evaluations of practical work graded on a pass/fail basis relative to her or his clock hour level of training.
Each student will be counseled and receive a progress report at 25% (226 hrs.), 50% (451 hrs.), and 75% (901 hrs.) of his academic year, as well as at the completion of his program. A student who is enrolled in the Nail Artistry and Teachers programs will be counseled at the midpoint of his course and at graduation.
Students are required to sign in on time. All students are on a clock hour program. The school rounds actual attendance hours to no more than the nearest quarter hour.
Only students who meet the minimum requirements for attendance and academic progress shall be considered to be making satisfactory progress until the next scheduled evaluation.
MINIMUM STANDARD PROGRESS POLICY...
Counseling sessions are between the student and his teacher. Discussions will include each student's strengths, weaknesses and methods of improvement. Students will have the opportunity to supply input on their training and the overall program. All counseling sessions will include an evaluation of the student's academic and attendance progress. The student and the teacher who is conducting the session will both sign the counseling records. A student who meets the minimum standards as defined in the publication will be deemed to be making satisfactory progress until his next scheduled evaluation. A student not in compliance with the school's minimum standards will be on probation until the next evaluation.
A student not making satisfactory progress can request probation, if his progress is borderline, and/or he can show that he can and will meet minimum progress standards in the next period. Students who are granted a period of probation will be considered to be making satisfactory progress until the next evaluation point and be eligible for Title IV assistance. If the student is making satisfactory progress at the end of the probationary period the probation will be terminated.
A student of probation who does not meet the minimum standards of satisfactory progress by his next evaluation point will be withdrawn from school.
A student for whom the school has cancelled his enrollment for not making minimum satisfactory progress may reenter the program after 30 days if he can show intent and the ability to meet the academic and attendance progress standards as defined in the publication. Students choosing this option will be required to enter into a new contract with the school for the amount of hours required to graduate.
A student who is meeting the requirements of the Satisfactory Progress Policy may request a Leave of Absence for any reason he or she deems and important reason. Requests for a leave of absence must be in writing on a school form or any form they choose. If because of circumstance a written request cannot be obtained, a phone request will be allowed. In either case the student's folder will be noted. A leave of absence will be granted for a period of no longer than 180 days, and the period will begin with the student's last day of physical attendance. A student who does not return to class by the last date granted by the leave of absence will be considered withdrawn. Leaves of absence are granted by the school, however all federal regulations will be followed. The leave of absence will extend a contract and the maximum time frame by the same amount of days of the leave of absence.

Hair had been the center of attention since people first began to care about their appearance. The beautification of an individual's nails and skin also factor into this concept. COSMETOLOGISTS, also called hairstylists, shampoo, cut, style, color, wave, straighten and generally fashion hair to enhance a person's appearance. They also create special effects using one or more of the mentioned techniques. Cosmetologists also manicure and artificially create and sculpt a person's nails to beautify the appearance of the hands. They perform scalp and facial treatments as well as apply facial make-up and pg 14 provide make-up analysis. They may work in a salon, or become the owner of a salon. They are also in demand today in full service spas. They are qualified to work as a nail tech, or branch out into the field of make-up artistry. NAIL TECHS, perform all services required to shape, sculpt and beautify a person's nails, hands and feet. They may work on nails in a nail salon, full service salon, spa, or with a nail service or nail product company. COSMETOLOGY TEACHERS perform all of the techniques described above and also teach in a licensed beauty school. COMPLAINT PROCEDURE... Any student who would like to express or register a complaint or grievance regarding any aspect of the instructional program, may do so with the teacher in the area. All teachers who receive a complaint or grievance will bring the same to the attention of the school's owner or director. Teachers will make every effort to resolve the situation to the student's satisfaction, as will all staff members. While manicurist are currently only 10% of the current industry employees (up from 2.6% in 1999), some 16% of the anticipated vacancies are for professionals with those skills.
APPEAL PROCEDURE...
A student may appeal a negative satisfactory determination. The school's director or supervisor and the student's teacher will hear appeals. The student will be given every opportunity to show cause why he feels his negative progress determination should be reversed.
Students who were receiving Title IV assistance before being withdrawn from training, due to a
negative satisfactory progress determination, or have been granted a leave of absence will have this aid reinstated if (a.) they have prevailed upon appeal, (b.) have reentered after an approved leave of absence, (c.) have reentered training and have reestablished satisfactory progress.
Course incompletes, repetitions, and non-credit remedial courses do not apply to cosmetology education; therefore they have no effect on the school's minimum satisfactory standards.
The satisfactory academic and attendance policies adopted and/or developed by the school, and contained herein, are for the benefit of those pursuing a career in cosmetology and related professions. This policy effectively ensures each student is making measurable progress toward the completion of the course of study, and will be a marketable member of the profession.

The salon industry is a job-seeker market. Some 56% of the salon owners reporting that they had job openings, More than 572,000 open positions were filled in 2002 with both experienced salon professionals changing positions (73% of the new hires) and with inexperienced professionals with less than one-year experience (27% of new hires). The supply does not seem to be keeping up with demand. Approximately 3 out of every 4 salons who looked for new employees in 2002 reported difficulty in finding qualified personnel. There is currently a "zero unemployment factor" in the salon industry.
